Brake Shop Clinic Feedback & Outcome Measurement

It is important for us to determine whether or not our educational programming is effective in helping our community partners and our catchment’s population to learn about and deal effectively with Tourette Syndrome and its associated conditions. The following outcome measurements are intended for use in determining that.

Received services from us? Please download our clinic’s satisfaction questionnaire here. Print, complete, and send to: Brake Shop Clinic, Client Satisfaction, CPRI, 600 Sanatorium Road, London, ON, N6H 3W7
Plan to attend our clinic’s ‘Leaky Brakes 101’ course? Please complete our Tourette Syndrome Knowledge Survey document both before the course (i.e. before seeing or listening to Session One), and immediately after it (i.e. at the end of Session Six). Please return these measures to our clinic, indicating which is the “Session One” measure and which is the “Session Six” measure. Others simply interested in testing their knowledge on Tourette Syndrome are also welcome to complete the survey.
Plan to receive a Presentation from our clinic? Please complete our Education Evaluation form immediately after the presentation. Please return this evaluation to our clinic.
Plan to receive a Peer In-Service from our clinic? You will receive a School Ratings of Student Disruptions to ‘Leaky Brakes’ Pre-Measure as part of the Peer In-Service Booking Form. Please now complete a School Ratings of Student Disruptions to 'Leaky Brakes' Post - Measure form (to be completed by both you and the child/youth) to rate outcomes one month after this Peer In-Service assembly has taken place. Please return this measure to our clinic.
Plan to receive a Training Workshop from our clinic? Following your attendance at one of our workshops, please complete the Personal/Professional Learning Contract to monitor how your own practice has been enhanced by attendance at this Training Workshop:
-Commit to a strategy in the space provided on the contract.
-Seal your contract in a stamped envelope return-addressed to you, and mail this package to our clinic (address on form).
-In two months time we will mail your return-addressed envelope back to you. Read your commitment again.
-Rate how well you followed through on this strategy on the self-assessment scale at the bottom of the contract.
-Mail us your self-assessment ONLY (detach from your learning contract to retain your confidentiality).
